UNFI Tank Exchange Agreement Forms

You are currently enrolled in the UNFI program with Blue Rhino. Blue Rhino and UNFI now require signed individual tank exchange agreements to continue to do business. The new agreement precludes any prior signed agreements.

To complete this requirement and ensure there are no disruptions in your service, please complete the following ASAP:

To Complete Online:

1. Download and complete your state's tax form:
2. Complete the Tank Exchange Application Online and attach your completed state's tax form:
3. Notify Blue Rhino Sales, that completed forms have been uploaded.

To Complete Offline (download) and Upload:

1. Download and complete your state's tax form:
2. Download and complete the Tank Exchange Application:
3. Upload all completed forms via our secure site:
   
   Notify Blue Rhino Sales, that completed forms have been uploaded.
